New Patient Forms
To make your visit as simple and convenient as possible, please bring:
- The below new patient forms
- Your referral form from your general dentist
- And any dental x-rays you've taken in the past six months
If you don't have them, we can take new ones. Please also bring:
- Any relevant information about your medical history
- Any medication you might be taking
- Your dental history
- And any dental insurance you may have
Please allow about 1-1½ hours for your first appointment. For your convenience just click on the link below, print the forms, complete and bring them to your appointment.
